Torrent Falls Climbing Adventure

EVERY person that participates in any/all of our activities needs to sign our waiver. All pages must be complete in order to climb. ***Participants of the via ferrata must be 10 years of age or older.*** The permission form is signed by the youth and also a parent or guardian. YOUTH WILL NEED ALL THREE PAGES FILLED OUT, THE PERMISSION FORM IS NOT ENOUGH. Also, please put the date that you will be climbing in the "climb date," please do not put the date that you are filling out the waiver. The contact information in "case of an emergency" can not be someone that is participating in the activity with you. Furthermore, if you make a mistake, just draw one line through error and initial correction. Please download the forms and fill them out prior to arrival.
